How to Say "I'm Not Feeling Well" in Malayalam

In short: To say you're unwell in spoken Malayalam, say എനിക്ക് സുഖമില്ല (enikku sukhamilla). For specifics: എനിക്ക് പനിയുണ്ട് (enikku paniyundu) "I have a fever," തലവേദന ഉണ്ട് (thalavedana undu) "I have a headache."

When & how to use it

Enikku sukhamilla is the general "I'm unwell." At a clinic, name the symptom with ...ഉണ്ട് (undu), "I have...". Ask about medicine: ഇത് ദിവസത്തിൽ എത്ര തവണ? (ithu divasathil ethra thavana?) — "how many times a day?".
